ChroniCare

ChroniCare is a human-centered design project to improve the healthcare experience for chronically ill patients. As a chronic pain sufferer, I’ve experienced firsthand how the medical system can be a burden to wellness. For my capstone class, I spent a semester researching, conducting interviews, and developing real-world solutions to make healthcare more accessible and compassionate. The system includes a redesigned medical intake form, an intuitive patient-facing notes interface, a reimagined pain scale, and accessible medication packaging. Through ChroniCare, I aimed to bridge the gap between patients and providers, creating solutions that better the lives of my community—and everyone.

Medication Bottle

Medication bottles haven’t been updates since the 1970’s, and I realized that they do not reflect our modern way of life, they aren’t as accessible as they could be, or as efficient. That goes for the labels as well, readability should be a main concern, but it seems to slip through the cracks with medication bottles. I modeled my design after a tictac container, or a lighter. Keeping my human-centered lens I created the ChroniCare’s own medication container. 


Symptom Map

My original plan was to redesign the intake form for a general health appointment, but after researching I realized that what was needed was a supporting tool to help make an average appointment more effective and efficient. Having a symptom map ready when you come in, with the icon system being used, to depict your chief concern of the visit will set priority for the visit saving time, and create a visual map to refer to during and after the appointment. This could be used virtually or on paper depending on patient preference. To bridge the gap and achieve better healthcare communication this is the perfect tool.

Demonstrating human-centered design I considered those with limited limb dexterity, this design can be used with one hand.

The directions on the front are clear, simple, and have a readable type that is more accessible.

The back has the easy to access information for any pharmacist to understand, including the necessary QR code for identity confirmation. 

The design fits into the ChroniCare brand seamlessly, bringing ease and comfort to healthcare.


Live Treatment Plan

The most popular solution that I came across with my research was some sort of personal portal that could connect providers, create clear, concise, and current plan to reference and use across all aspects of ones health. This product illustrates human-centered design at it’s core. For those with chronic illness keeping track of your treatments, and appointments can be a huge part of managing, this model would bring it all in one place, would be interactive, and user-friendly.
